| Psychosocial Rehabilitation Program
Philosophy of Program
The Psychosocial Rehabilitation (PSR) Program is designed to serve individuals with impaired functioning who would benefit from a structured program providing curriculum-based services.
Services are designed to support the recovery and/or stabilization of the individuals. Through service provision, symptoms will be reduced and individuals will experience an improvement in level of functioning. The PSR Program is structured to improve the ability of individuals served to reside in the community. The program strives to continually improve service provision in order that individuals served experience an enhanced quality of life.
Program Description
The PSR Program operates from 8:00 a.m. to 4:00 p.m. Monday through Friday from the program building.
The PSR facility has been designed to provide complete access to physically disabled individuals. The agency endeavors to accommodate individuals with special needs, or refers to providers equipped to meet their needs.
Admission Criteria
The PSR Program provides services to individuals who meet the following admission criteria:

18 years of age or older
|
•
|
resident of Brown, Cass or Schuyler County |
•
|
a primary diagnosis of a psychiatric disorder |
•
|
limited independent living skills |
•
|
do not require a skilled level of care |
•
|
are behaviorally appropriate for a group-structured program |
Exclusionary criteria includes:
•
|
requires services that are not available to meet the needs of the individual |
The PSR Supervisor is responsible for providing the final determination for eligibility. Individuals determined ineligible will be provided with a referral to alternative services.
Discharge
Discharge criteria has been established consisting of the following:
1. Individual request
2. Violation of the program guidelines
3. Individual requires level or type of service not provided
4. Behavioral manifestations are disruptive to the stability of the other participants
5. Non-compliance with agency policies
Description of Services
•

Curriculum-based services designed to assist individuals with the following: understanding and coping with a psychiatric disorder; appropriate socialization and behavior; daily living skill acquisition; and educational/vocational development. |

Curriculum is provided in various methods of presentation including role play, guest speakers, trainer instruction/lecture, consumer instruction/lecture and hands-on training. |
•
|
Structured physical, recreational and arts/crafts activities. |
Program Guidelines
•
|
Consistent attendance to programming is required according to the individual treatment plan. |
•
|
Inconsistent attendance and/or unexcused absences will result in discharge from the program. |
•
|
Participation in all phases of the program is required unless excused for medical reasons. Individuals must present a signed nurse or physician order. |
•
|
Individuals must participate in all emergency or evacuation drills as scheduled. |
•
|
Obscene and/or abusive language or inappropriate behavior will not be tolerated. |
•
|
Threats of physical violence or physical violence will not be tolerated. |
•
|
Individuals are responsible for informing the PSR Supervisor of any prescription or over-the-counter medication brought to programming. |
•
|
No food will be allowed outside of the cafeteria. |
•
|
Individuals are responsible for disposing of all trash into the proper receptacles. |
Absences
It is the individual's responsibility to complete the Absence Request form and turn it in to the PSR Supervisor if anticipating an absence from programming. The following are considered excused absences:

illness or hospitalization |
•
|
individual and/or family crisis |
•
|
authorized family visits |
•
|
scheduled appointments (such as legal, medical, & social services, and other staff approved appointments) |
All other absences are unexcused and are considered a violation of program guidelines. In addition, no absence will be excused if an Absence Request form is not completed.
Individuals at programming who need to leave unexpectedly must receive permission from the PSR Supervisor prior to leaving. The supervisor will determine whether the absence is excused or unexcused and an Absence Request form will be completed together.
Behavioral Intervention
Violation of one or more of the program guidelines will result in the following actions:
1. For minor violations, staff will utilize the following intervention techniques:
A. Verbal redirection
B. 15 minute "time out"
2. In the event that the above interventions are not successful, the PSR Supervisor will hold a consultation with the individual to discuss compliance with program guidelines.
3. If the violation reoccurs, the individual will participate in a clinical staffing with the Treatment Team (and others, if appropriate) to take one of the following actions:
A. Assessment/referral for appropriate, alternative services
B. Discharge from the program
